Information for injured workers in North Carolina

Q: How do I document my disability?  What does that mean?

A: In order to provide written proof of your impairment, your doctor(s) will need to submit reports to the SSA about your condition.  Medical evidence of your disability or disease is vital to your case.  It is very important that you give them specific comments about how you are feeling at each of your check ups and that they write down the information in your medical records.  Do not just tell them you can't sit for very long, for example.  Tell them that because you are unable to sit for more than 10 minutes in an hour or that you have to stand while waiting for the bus to get to their office sometimes for more than 45 minutes and it is very tiring.  Tell your doctor also about your quality of life and what you could do before your disease or disability progressed.
